Volunteer Application Example

This is a copy of a volunteer application for a hospice society. I suggest using this as an example for ideas. All applications should be tailored to meet your agency's specific needs. Think about what information you need to know, why you need it, how you will use it, and whether you need it up front or if it is something you will gather after accepting someone as a volunteer. There is no point in asking questions if you aren't going to use the answers.
